Death, Dying, and Souls

Lore?

When a creature native to the material plane dies, its soul is released from its body as a blue spirit flame. These soul flames remain on the material only briefly, visible only to a gifted few, before fading away to the border ethereal and then to the Land of the Dead.

This demiplane is said to exist at the centre of all creation, overlapping the Ethereal plane and the Shadowfell. The walls between the Land of the Dead are thin in some areas, and this demiplane is directly fed by the Plane of Negative Energy.

Souls awaken and quickly find a path. The paths twists and turns and has many forks. Minion's of the Raven Queen may attempt to swoop in and steal souls from the path. Fiendish envoys attempt to entice souls to follow them.

Death may be certain, but its course surely isn't.

Hopefully before succumbing to these temptations, an Aspect of Death will find the creature's soul to accompany them on their journey. The temptations of demons and devils may continue, though @"Aspects of Death" often banish many soul snatchers in their way.

If the soul arrives at the Metropolis of Souls (or City of Souls?), they meet no more resistance, and continue their way to the Palace of Bones. The soul must linger within the palace until its time of judgement arrives. When its time comes, the soul is beckoned into the audience chamber housing the Court of the Dead is housed.

At this point a soul is weighed upon the scales, and its final destination is determined by its weight and by any claims made to it which are presented to the Court.

It may be the home of its deity, it may be taken by one that claims it, or the Court may lay claim to the soul themselves.
